FOR WORKING WITH ALYSSA

By continuing with the agreed upon service, your consent and agreement with the policies and procedures below, unless otherwise specified, is implied.

POLICIES AND PROCEDURES 

 

ELIGIBILITY

A referral is not required. Service continuation is at the clinician's discretion. Services by Alyssa are only offered at this time to residents of Ontario, Nova Scotia, Prince Edward Island, the Northwest Territories and the Yukon. 

 

SESSION TYPES, DURATION AND RATES

All rates are set in accordance with the Ontario Association of Speech-Language Pathologists and Audiologists guidelines. 

Services and rates are subject to change.

 

Assessment Session

  • An Assessment Session is required prior to all Therapy and Training Services for a) New Clients and b) Existing clients who have not attended a session within the last 12 months. 

  • An Assessment Session is one-to-one, online, and is approximately 50-60 minutes in length. 

  • The rate for an Assessment Session is $175. This includes one-to-one session time as well as post-session analysis and program planning by the clinician. 

 

Standard Session 

  • A Standard Session is one-to-one, online, and is 50 minutes in duration.

  • this training and/or therapy session is available, upon clinician discretion, to existing clients who have already completed an assessment within the last 12 months. 

  • The rate for a Standard Session is $175. This includes one-to-one session time as well as planning, charting and analysis by the clinician. 

  • Each Standard Session includes one optional and complimentary Online Check-In. Online Check-Ins are approximately 10 minutes in duration and provide a between-session review of previously established exercises. Online Check-Ins may take place online, over phone or email, or may be an independent review of samples shared by the client. Online check-ins must be used between sessions and do not carry forward. 

 

Half Session

Half Sessions are available only upon clinician discretion and to existing clients who have completed a Standard Session within the 4 weeks prior.

  • A Half Session is one-to-one, online, and is 25 minutes in duration. 

  • The rate for a Half Session is $93. This includes one-to-one session time as well as planning, charting and analysis by the clinician. 

 

Online Consultation 

  • An Online Consultation is available to potential clients who wish to meet with Alyssa and trial a virtual online meeting.

  • An Online Consultation is not an assessment. Professional recommendations will not be provided without assessment. 

  • An Online Consultation typically 10 minutes in length but may extend up to 15 minutes. 

  • Online Consultations are available free of charge.

 

MISSED APPOINTMENTS AND CANCELLATIONS

Missed Appointments and Cancellations with less than 24 hours notice will be charged for the full value of the appointment when without reasonable cause (e.g. emergency, illness). To cancel, call or email your clinician or follow the Appointment Cancellation link in your appointment confirmation.

LATENESS

We understand that lateness happens from time to time! If you suspect you may be late to your appointment, please email or call your clinician to let them know. With notice, your clinician will do their best to find ways to make up for missed time.

 

If the client arrives late to the appointment and before the half session mark (25 minutes for a standard session or 15 minutes for a half session) the session may continue for the remainder of the scheduled duration. 

If the client does not arrive before the half session mark (25 minutes for a standard session or 15 minutes for a half session) the session is deemed a Missed Appointment. The appointment will be billed for the full value of the originally scheduled appointment.

LOCATION OF SESSIONS

Only Online Services are available at this time. 

 

Online Sessions take place through your browser via Cliniko, our Telehealth medical record platform. You do not need to download anything to use Cliniko, but it's recommended to update your browser. Simply follow the link in your appointment confirmation and appointment reminder emails at the time of our session to join.

 

Online Sessions may take place via Zoom by request. Should you prefer to meet via Zoom, please contact Alyssa by email to receive the appointment link. 

PAYMENT

Payment is due at the time of each session. Please allow for up to 5 business days for your payment to be processed. Once your payment is processed, a receipt will be issued with all information required for your records or for you to submit an insurance claim.

 

Unless otherwise arranged with your clinician, if payments haven't been submitted within 13 days following an appointment, further sessions will be paused and  a late fee of $50.00 will be applied.

Currently, e-Transfer is the preferred method of payment for online sessions.

Should you require an extension for payment, please notify Alyssa in advance so arrangements can be made in order to avoid late fees.

COVERAGE

Check with your insurance provider to find out more about your coverage options, eligibility, and requirements, as this information is not available to Alyssa. Most private health insurance plans offer coverage for speech-language pathology or speech therapy services. Clients are responsible for the payment of services rendered at the time of the appointment. Once your payment is processed, you will be provided with a receipt to submit for reimbursement. The client is responsible for any balance that is not reimbursed by an insurance company. Private speech-language pathology services in Ontario are not covered by OHIP.

FEES

Clients will be made aware of any fees for the services they have agreed upon prior to service provision. Changes in fee structure may apply for promotions, concessions, or group services. Additional services, including but not limited to, report writing, phone consultations, travel, and consultation with other professionals, are subject to additional fees.

 

CONSENT

By continuing with the agreed upon service, your consent to receive service, agreement with the policies and details below, unless otherwise specified, is implied. A signature is not required. Consent is required to continue with services.

Your participation is voluntary. You have the right to discontinue services at any time. You have the right to withdraw your consent and participation at any time. If you wish to withdraw consent, please provide written notice mailed to Alyssa McCarthy (Speech Appeal), Internal: 2300 Yonge Street; Suite 1600, Toronto, Ontario, M4P 1E4.

 

PRIVACY AND CONFIDENTIALITY

The collection, use, and disclosure of your personal health information (PHI) is collected and stored to the best of my ability in accordance with the recommendations by the College of Audiologists of Speech-Language Pathologists and Audiologists (CASLPO) and by the Personal Health Information Protection Act (PHIPA).  Alyssa McCarthy is required, as per CASLPO and PHIPA regulations, to collect client PHI. You, the client, are responsible to provide accurate PHI and to notify Alyssa McCarthy of any changes to PHI. Please do not transmit sensitive information electronically. Despite efforts to protect the privacy and security of electronic communication, it is not possible to completely secure the information. Use of electronic communications to discuss sensitive information can increase the risk of such information being disclosed to third parties. Online services may have a legal right to inspect and keep electronic communications that pass through their system. Email, text messages, and instant messages can more easily be misdirected, resulting in increased risk of being received by unintended source, and may also be easier to falsify than signed hard copies, as it is not always feasible to verify the true identity of the sender. During Online Sessions, clients are recommended to input their PHI directly into the HIPPA and PIPEDA-compliant medical record system, Cliniko, for safe sharing and storage of sensitive information. Communication during In-person and Online Telehealth Sessions and outside of sessions may be copied or recorded in part or in full and stored securely as part of your clinical chart. Communication outside of the clinic occurs as per your authorization. Financial, personal and clinical assessment and therapy information will not be disclosed over the phone, email, or in hard copy without your verbal consent and authorization. In compliance with PHIPA, you have the right to request access to your personal health information. All requests must be made in writing via mail to Alyssa McCarthy (Speech Appeal), Internal: 2300 Yonge Street; Suite 1600, Toronto, Ontario, M4P 1E4. Records will be made available within thirty days.

 

DISCONTINUATION OF SERVICE

You have the right to discontinue services at any time for any reason.

Alyssa McCarthy may discontinue services if:

  • it is determined that therapy is no longer appropriate

  • there is no significant progress after a reasonable amount of time

  • continuation poses a threat to health or safety of client or clinician​
